The department began functioning as a service for out patients in Sector 22 Polyclinic on 4.10.1993. It began providing indoor facilities as well as emergency services to the patients in Sector 16 General Hospital, Chandigarh. Inpatient services in the present campus in Sector 32 were started on 23rd March 1996 and Emergency Services on 26th March 1997. OPD services were shifted to Block-B in Govt. Medical College Hospital, Sector-32 subsequently. The department started with six Senior Resident and as it developed, the strength has increased to a sanctioned strength of 15 and an equal number of Junior Residents over the years to cater to the increasing academic needs & patients care.  The department caters to an average of 250-275 patients per day in OPD and has nearly 100% bed occupancy in the emergency and general wards. There has been a consistent increase in the number of patients attending the various services of this department. Over the years various specialized services like upper and lower gastrointestinal endoscopy, endoscopic retrograde cholangiopancreatography (ERCP), hemodialysis, neurophysiology services like evoked potentials, EEG, noninvasive cardiac tests like treadmill testing, Holter monitoring and echo-cardiography, solid organ biopsies, intracarpal injections, joint aspiration, intra-articular injections have been provided to patients. Recently a new facility has been launched through “e-Sampark services” by which OPD registration can be done online. The department of General Medicine has been providing high quality medical services to the people of Chandigarh and its surrounding areas from Western UP, Himachal Pradesh, Haryana and Punjab. It is also actively engaged in academic, teaching and research activities, which are an essential part of any good medical college. 

The department of medicine was started in 1992 and was first headed by a very eminent internationally renowned neurologist Prof J.S. Chopra, MD, DM (Neurology) Padma Bhushan who was also the Founder Director Principal of this institute. 

The department of medicine in August 1994 got its second head in Prof. H.S. Malhotra, MD, DM (Nephrology) a nephrologist from the PGIMER, Chandigarh. His extensive experience in computers, lead to a multifaceted development of not only the department, but also the college as a whole, especially in the fields of computerization and telecommunications.

Dr. Atul Sachdev MD, DM (Gastroenterology), MAMS presently heads the department after having received his postgraduate training in medicine and gastroenterology at PGIMER Chandigarh. He is a gastroenterologist and an internist, who has been on the faculty of General Medicine since September 1993. He has been the pioneer of gastroenterology services in this institute, which the department has been providing since 1994. The department provides high quality diagnostic and therapeutic endoscopic services to a large number of patients of the city and surrounding areas. Recently he has been awarded membership of National Academy of Medical Sciences. He is the founder member and secretary Gastrointestinal Infection Society of India and currently is President elect of Gastrointestinal Infection Society of India. He has several national and international publications to his credit and reviewer in various journals of repute including Journal of Pancreas, Emergency Medicine and Journal of Neuropsychiatry and Neurophysiology, Hepatology, Neurology India.

Dr. Ram Singh, MD is Professor in the department since 2004. He has been trained as an internist at PGIMER Chandigarh along with 3 years experience in Hepatology. In addition to being a good internist, he is also trained in neurophysiology, dialysis and endoscopy. He however spends most of the time looking after the neurophysiology laboratory services i.e. EEG, EMG, NCV. He has a special interest in diabetes mellitus and is a member of Academic Branch of Research Society for Study of Diabetes in India. He has several National and international publications to his credit. He is also the deputy medical superintendent of GMCH for the past 6 years.

Dr. S S Lehl, MD is Reader in the department since 1999. He has worked in the department of rheumatology in PGIMER for three years. He has special interest in the field of Geriatric Medicine. He was a member of working group in a WHO workshop on health care of the elderly.  The department of Medicine is the only department which is running a specialized clinic in geriatrics in this part of the country under his stewardship. He is on the national advisory board for the Journal of Indian Academy of Geriatrics. He has been co-investigator in 2 WHO sponsored projects related to health care of the elderly. He is also involved in non-invasive cardiac care looking after Tread mill test laboratory. He has several National and international publications to his credit. He has been elected as the honorary secretary of Indian Academy of Geriatrics

Dr. Sanjay D'Cruz, MD, DNB, DM (Nephrology), MAMS received his postgraduate training in medicine and nephrology at PGIMER Chandigarh. He joined as Senior Lecturer in 1998 and was promoted as a Reader in 2006. He is credited to have started the nephrology services in this institute including general nephrology, renal biopsies, emergencies in nephrology and a dialysis unit, which is currently catering to the needs of patients with acute and chronic renal failure. He runs the renal clinic in the outpatients department. He has been awarded scholarship by the International Society of Nephrology in the year 1997. He went as a visiting professor in the faculty of medicine to Jianxi Chinese Medical University, Peoples Republic of China in the year 2007 for a period of 4 ½ months. Recently he has been awarded membership of National Academy of Medical Sciences. He has several National and international publications to his credit. He is also a reviewer for various journals including Nephrology, World journal of Nephrology, World journal of Pediatrics, Journal of Pancreas and Indian Journal of Nephrology and Journal of Metabolomics and System Biology.

Dr. Monica Gupta, MD, DNB Medicine trained as an undergraduate and post graduate in medicine at the All India Institute of Medical Sciences (AIIMS), New Delhi joined as senior lecturer through UPSC on 11th April, 2005. She has special interest in the field of Diabetes and Geriatric Medicine. She runs the Diabetes and Metabolic disease clinic in the out patient department. She is involved in non-invasive cardiac care looking after Holter laboratory. She is a member of various prestigious and professional organizations and has published several papers. She has been elected as the Treasurer of Indian Academy of Geriatrics

Dr. Jeet Ram Kashyap, MD trained as an undergraduate and post graduate in medicine at the Indira Gandhi Medical College (IGMC), Shimla. He joined as a senior lecturer on 21st July, 2006. He has been trained in non-interventional cardiology at GB Pant hospital, New Delhi. He has special interest in cardiology and looks after non-invasive cardiology services including echocardiography. He runs the Cardiology clinic in the out patient department. Currently he is on study leave  persuing DM (Cardiology) course at AIIMS, New Delhi.

Dr. Pritam Singh, MD is the newest faculty member to join the Department of Medicine, GMCH. He received his educational training as an undergraduate and post-graduate in General Medicine at Maulana Azad Medical College (MAMC), New Delhi and joined as a Assistant Professor in Department of General Medicine, GMCH on June 24th, 2011. He has an academic interest in the field of Critical Care Medicine and is actively involved in management of patients in Emergency ward, including patients requiring mechanical ventilation support. He has several academic publications to his credit.

In addition to Internal Medicine service the department is providing specialized services in the following fields: Gastroenterology, Hepatology, Cardiology, Neurology, Rheumatology, Nephrology, Diabetes, Thyroid, Haematology & Oncology & Geriatrics. It is worth mentioning that the first Geriatric clinic in this region was started by our department to cater to the needs of the growing elderly population. 

The outpatient clinics are situated in Level - V, "B" Block, Area 53 in the OPD Block. Geriatric clinic is situated on the ground floor of B" Block in the Room No.1405 for the convenience of the elderly patients.

Gastroenterology Services:
Upper GI and Lower GI endoscopy are being done since the inception of the department. Therapeutic endoscopies were started in 1994. ERCP was started in the year 1995.   Available equipment includes state of the art video endoscopes. Recently the department of Medicine has procured its own "C"-Arm for ERCPs'.

Neurology Services:
The services of EMG, NCV, EEG and Evoked Potential are being provided since the year 1995.

Cardiology services: Non invasive cardiology services like Tread Mill Testing (Stress test), Holter and Echocardiography are being provided since 1995. We have a state of the art 9 bedded Coronary Care Unit (CCU) situated on level 5, block C. The department plans to start 24 hours ambulatory BP monitoring facilities in the near future.


Nephrology services:
Hemodialysis was started in the year 1998. Five Hemodialysis machines cater to patients with acute and chronic renal failure and toxin overdose. Hemodialysis for inpatients is regularly performed at our center. Facilities for peritoneal dialysis and continous ambulatory peritoneal dialysis (CAPD) are also available in the hospital. Renal biopsies are done regularly as a diagnostic tool.

Hematology services: Full care to patients with hematological problems including anaemia, thrombocytopenia, neutropenia, leukemia and lymphoma is being provided to the patients.

Rheumatology services: Patients having various rheumatological problems are provided comprehensive health care. Facilities and expertise for intra articular injections, muscle biopsies, sural nerve biopsies, minor salivary gland biopsies, intra-carpal injection and immunosuppressive therapy is available in the department. 

 

RESEARCH & TRAINING

The department provides quality clinical and theory teaching to undergraduate MBBS students and junior residents. The department is involved in teaching of 200 students/ year of four batches. The Medicine curriculum starts form the 2nd year and continues till the end of final year. Both didactic lectures and clinical demonstrations are held and on an average the department takes about 8-10 theory lectures/week and 18 clinical demonstrations/week. Symposia and tutorials are held regularly for the students and their performance is assessed at periodic intervals in both theory and clinical aspects. First batch of MBBS students graduated in the year 1996. 

Faculty members of the department participate in various national conferences and present scientific papers and deliver talks at different forums. Over 80 papers have also been published by the department since its inception in a short span of 10 years in leading national and international journals. The department has been organizing CMEs regularly since November, 1998. This activity has become an annual feature in the academic calendar of department. The department has been organizing six monthly workshops for the Punjab Health System Corporation. It has also been involved in conducting WHO sponsored workshops for training medical officers in health care of the elderly. Research activities are being actively pursued by the department in various fields.

3.  Workshops Organised by the Department of General Medicine, GMCH

(i) Workshops Organised for Punjab Health System Corporation (PHSC).

Regular workshops for doctors from PHSC have been conducted over the years including topics like Cardiovascular, Respiratory, Gastro-intestinal & Endocrine Emergencies. A one-month training programme for general duty medical officers was organized from April -May 1998 with other clinical departments.

(ii) Workshop on Medical Education technology.

A two-day workshop on "Concepts in Medical Education technology" was held on 10-11 February 1998. Dr. V S Mathur from university of Bahrain was the guest speaker. The faculty heads were introduced to concepts in medical education technology.

(iii) WHO sponsored National Board of Examinations Workshop. 

It was organized the topic “On Evaluation Training and Research Methodology” in February 1999 in association with the Medical Education Cell of GMC, Chandigarh.

(iv) Workshops on Care of Elderly.

- WHO sponsored two workshops have been organized on training of medical officers in health care of the elderly in 2003 and 2005.

- WHO sponsored multicentric study to establish epidemiological data on health problems in elderly Indian population and development of     strategies to address these.

- WHO sponsored project on “Community based care of the elderly”

(v) Workshop /Training /Sensitization on Rheumatic Fever / Rheumatic Heart Diseases – July, 2008

(vi)  Conducted workshop / CME on Rheumatology for Undergraduate students - 21.08.2008.

4.  Preparation of a documentary film on Objective Structural Clinical Examination (OSCE).

This OSCE film was made in collaboration with National Board of Examinations, New Delhi in 1998 and since then been circulated to all medical colleges and centers where Diplomat in National Board courses are being conducted.

5. Faculty from the department have been involved as part of the MCI inspection team for inspection of various medical colleges for undergraduate, MD and DNB courses

6. DST projects :

(i) The evaluation of cardiovascular risk factors in 30-45 years old employees of Chandigarh.


(ii) To study The Relationship of Body Iron Stores with Type 2 Diabetes And Obesity in North Indian Population

(iii) Pattern of Co-morbidities in elderly patients(more than 65 years) of chronic obstructive pulmonary diseases and outcome assessment in those hospitalized with these morbidities.

8. Ongoing Projects

1. WHO project on Community based care of the elderly.

2. Department of Medicine was part of  the "Multicentric study to assess the problems of the Elderly and to develop strategies to address these" of which the department of Medicine was one of the 10 centres from all over the country was also compiled by the Department of Health and Family Welfare, New Delhi.

3. Department of Medicine is part of imparting training to students as part of Prehospital Hospital Trauma Technician Course (PHTTC)

1. The department of Medicine completed a WHO sponsored project entitled "Multicentric study to assess the problems of the Elderly and to develop strategies to address these". The department was one of the 10 centers selected from all over India for conducting this study. 

2. The teaching facilities of the department have been upgraded by adding Laptops, Computers and a Multi Media Projector facility from departmental funds to upgrade teaching methodolgy. 

3. The department has been designated a centre for training of Doctors in Health Care of the Elderly in this region by WHO.

4. The department is the coordinating hub for the telemedicine project under the chairmanship of Prof. Atul Sachdev. To begin with we are having simulcast of weekly CPC from PGI at 8am on every Wednesday.

5. New Holter System for analysis of arrhythmias in the cardiac patients in the year 2009

6. 24 hours ambuilatory blood pressure monitoring in the year 2010

7. Four Ventilators installed in the department of Medicine for critically ill patient management in 2009

The department plans to become self sufficient in all medical specialties to reduce the need for referral of patients to other centers as well as continue to provide high quality comprehensive services to the public.

As part of its CME programme, which is held regularly, the department will continue to strive to educate the physicians of the region so that they are kept up to date with the latest developments in medical field.

The department will continue to improve upon its curriculum and teaching methodology to make the outgoing graduates capable of meeting the challenges in general practice. The department plans to start educational programme for the public in certain key areas. The department hopes to start postgraduate courses very soon. A proposal to have super specialists in all the medical specialties is in advanced stage of implementation and the department will soon have specialists in Neurology, Endocrinology, Cardiology in addition to the existing specialists in Gastroenterology and Nephrology.

The department, in spite of it preoccupations with patient care, training of undergraduates and setting up of this hospital has already been participating in various conferences and presenting and publishing papers in international and national journals.
